Patient and Family Advisory Council
Highpoint Health – Sumner with Ascension Saint Thomas is a strong presence and supporter to our communities and is dedicated in making them healthier. In order to hear our community members’ voices, we have created the Patient and Family Advisory Council (PFAC). Each member will bring a unique experience and a positive attitude to the table as we help improve the quality of our hospital’s care for all patients and their family members.
Our PFAC council is made up of volunteer members of our community who were former patients or family members, and who will partnership with bedside staff, physicians, and hospital leaders. This team will provide a voice that represents all patients and families of patients who receive care at Highpoint Health—Sumner with Ascension Saint Thomas.
You can be an advisor if:
· You or a family member received care at Highpoint Health—Sumner with Ascension Saint Thomas in the last five years
· You are willing to tell your experience story
· You are willing to participate in meeting discussions
· You are willing to review education, information materials, and scoring data
· Can meet monthly for 1-2 hours and commit to a two- or three-year membership
· Will support our hospital team and the continuous efforts to improve
The PFAC Team will help Highpoint Health—Sumner with Ascension Saint Thomas improve the patient and family experience and deliver the best patient outcomes by being a champion of Sumner patient care. Together we truly make a difference.
